Below you will find the forms needed to become a volunteer at the Language Academy: 

 

​2025-26 Volunteer Application (English)

Please fill out the application forms and turn them in at the main office along with the following:

• Valid Government ID with a Picture (Driver's License, State ID, Military ID, etc.)

• A copy of a TB (Tuberculosis) Test :

  • Skin or Blood tests, anything within the last 4 years is still considered valid

  • Chest X-rays are valid for 7 years

  • Signed Risk Assessment test. This can be signed by your provider or the School Nurse (this is only on days that she announces)​
